Opengarage and Modern Forms Integrations are broken. Both work fine with 2025.5.3. The Modern Forms Integration says that the api header is wrong and the Opengarage integration cannot find the controller.
Then you should open issues in GitHub. You’ll need to post logs though (which you haven’t done here).
