Make sure your zwave stick is on a USB extension cable away from the USB ports, away from metal server chassis, and not inside an enclosed metal server rack.
Metal is bad for radio waves.

By the powers of me I shall resurrect this dead thread! Have you found a solution for the failed health check? I too am having the same issues. The zwave device works but the health checks failed just like yours.
What kind of device is it? Battery operated sensors go to sleep. They only wake when they’ve got something to say, or when you wake them manually. Don’t ping us, we’ll ping you
