Creating a Modern Pager - Vibrating Wearable

I’m looking to create the modern equivalent of a pager - a wearable that vibrates repeatedly when triggered by a Home Assistant automation or notification and keeps going until I acknowledge the alert on the device. Ideally would like this be something like an Android/Wear OS watch but am open to other wearables.

I was able to accomplish something similar on my phone via Macrodroid but have not been able to definitively determine if the equivalent is possible with a smart watch.

1. Home Assistant sends a notification to smart phone containing specific string of text.
2. MacroDroid detects the notification based on text content, starts vibrating the phone, and repeatedly plays a designated notification sound file.
3. Pressing the phone’s volume-down button stops the alert, stopping both device vibration and playback of the sound file, and dismisses the notification.

I’d like similar behavior on my wrist without having to have my phone in my pocket. Ideally I’d be able to press an on-screen prompt or physical button to dismiss the notification and stop the sound and vibration based alert. Key requirement is that the vibration continues until I deliberately acknowledge it.

Does anyone know if this is achievable? It’s a bit niche, I know, but it seems straightforward but I haven’t been able to find much online about anything similar. I don’t know if this achievable with something like Tasker + AutoWear, the Home Assistant companion app alone (which I doubt) or a different solution?

Appreciate any hardware and application recommendations folks might have.

I think Wear OS will connect with HA without the phone, but you need the phone to handle notifications.
