Just what the title says… Most of my Zooz devices have been added perfectly using smart start, but I have one Zen76 switch which appears to be getting “stuck” in provisioning with no node id assigned. Switch is powered. I’ve switched it on and off a couple times after trying smart start.
Try factory resetting the device sometimes they come from the factory included into a network.
Thanks meant to mention I’ve factory reset the switch a few times for good measure = same result.
I just restarted HA as well and it’s still sitting in provisioning…
I can add it no problem with a traditional inclusion, but not with smart start.
You may have to put the ZWaveJS-UI logs in silly mode and see if you ever get something like:Received Smart Start inclusion request when you power up the switch.
I myself don’t know how well Smart Start works with Network Wide Inclusion, so one thing to try would be to place the switch near your controller and power it up to see if that makes it work.
Z-Wave JS UI has a debug page which lets you see the logs in real time as they happen. I would exclude a know good device then re-include it while looking at the debug page then compare that to the device that doesn’t work.

I have simiar issue but in my case it is rather Z-wave JS UI making troubles - I can’t add device with Smart Start and secure inclusion is also not working (with the info that the inclusion is timed-out)
