Has anyone flashed with the official yaml from the voice PE github to only have wake words stop working entirely? If so, what was the solution to fix it?

Do you have default wakewordset in HA.

Also look at device info page and check settings there

Yes. I also tried changing the wake words. It will work with a button press but not with any wake word I set. The ESPHome logs also show the wake word being applied. It worked out of the box but doesn’t after flashing the yaml and I just can’t figure out why…

What wake word are you using?
Are you sure it hears you correctly? Try difference pronunciations.

Also. When you press button and say command the command is heard?

I was using “okay nabu” and it worked perfectly out of the box but stopped after I tried flashing the yaml from github. Interestingly, I followed this and it fixed it.
