iPhone shortcut will not toggle switch on one phone

I have an automation that opens the garage door using a shelly device.
Works like a charm.

I added an NFC tag to my phone (15 Pro) using Apple Shortuts and it calls Home Assistant using the “Control Switch” option.
The switch “Garagedoor” is selected, target state is off and it is set to “Toggle”

On my device, it works.
Configuring the exact same thing on my wife’s iPhone (13 Pro) doesn’t work.

I get the vibration that it scanned the tag, but nothing happens.
HA logs also show an interesting difference. On my phone it executes a toggle, on my wife’s phone it executes a “Turn on” instead, even though the automation is set to toggle.

Garagedeur turned on triggered by action Switch: Turn on
09:04:20 - 18 minutes ago - phone 2
Garagedeur turned off triggered by action Switch: Toggle
08:57:23 - 25 minutes ago - phone 1

I have no clue anymore how to troubleshoot this. Any ideas?

Well… problem solved it seems.

I rebooted the iPhone and about 20 or so automations fired off, all with a toggle.

Come on Apple, this isn’t a Windows machine, what’s up with the reboot requirement?

Well, the problem unfortunately is back.

Even after reboots, the shortcut does not “reach” home assistant.
It fires, the phone vibrates indicating it has performed the action, but nothing shows up in the logs anywhere. I am stumped.

HA works well from the phone
I can push the button in HA and it works

The only thing that does not work is calling HA from the shortcut.

Any ideas?
