Leviton dimmer keeps disappearing

I have a Leviton DZ6HD dimmer switch that seems to fail every month or so with the following message appearing on HA:

Device Was Factory Reset!
In-Wall 600W Dimmer has been factory reset and removed from the Z-Wave network.

I replaced it once about 2 years ago when the old one failed. Is anyone else having similar problems with these Leviton dimmer switches or could there be a HA compatibility issue?

I have the same problem two days ago. When yours does this can you add it again? I can’t seem to add mine so guess have to buy a newer model from Leviton.

I have to force the Leviton to search for a new host then add it through the HA interface and it re-appears.
