Zwave Supervisor Add-on - "Failed to start zwave add-on"

I’m a very new (but eager) Home Assistant user, so please bear with me.

The problem:
I’m attempting to add the zwave integration. I check “Use the Z-Wave Supervisor Add-on” and click Submit. I choose the only path that has a device populated and click Submit. It spins and eventually times out saying “Failed to start zwave add-on”. I can only include one picture per the rules since I’m new, I chose to attach a pic of the usb path screen rather than the error in case anyone can spot something that my newbie eyes can not.

What was tried so far:
I initially started out with a ZOOZ ZST39. When I first followed these steps with this device I was successful and had my Z-Wave devices added and working perfectly. After one week or so, the controller would show offline as well as all of my devices. I could not get the controller to come back online.

I eventually deleted the devices and removed the integration thinking I needed a fresh start. When I reinstalled the integration, my instance could no longer see the controller. It would simply not show up as a USB device when trying to choose a path.

After researching online and trying for over a week, I bought the Home Seer Smart Stick G8 thinking it might be the controller. At first, this new controller did not show up as a device when choosing a usb path as well, but last night I upgraded to the latest HA version. This leads us to where I am today with the error mentioned at the beginning. I now see a device to choose in the USB path which feels like a step forward, but I can’t proceed due to this error.

Any help is appreciated. This is my first post and I’ve been hesitant to post because I feel like this will turn out to be an easy fix, but I could definitely use some direction here as I am feeling defeated. Thank you.

What hardware is this? You say OS: 15 can you clarify that this is HAOS on bare metal? What specific device?

That device you are selecting in your screenshot doesn’t look like the Z-wave controller to me.

If you navigate in the menu to System > Hardware then click “All Hardware” can you find the Z-wave controller in the list of detected hardware?

Thanks for taking a look. Sorry, I should have added that it’s an old Dell P25t with Windows wiped and HAOS installed. I did take a look in the hardware section as you advised, I don’t see anything indicating a Z-Wave controller… Both usb ports are showing the same info in there. The usb stick has an led that is lit up.

This is the same usb port I had the zooz 800 originally plugged into and working on. Not sure of thats a problem in itself. I did try the other usb port and I noticed that the device path that’s in my Pic above is gone. That’s why I was assuming it’s the controller, or at least an acknowledgement that something is plugged in. I’m not sure where to go with it from here, so any input is appreciated. Thanks.

I’d recommend reaching out to HomeSeer support to see if the drivers needed are in HAOS 15. They can also tell you what the port will appear as under the Linux system.

Try powering off the PC completely, unplug the controller, plug it back in and turn it on. Check the Hardware page again. You can also try your old controller and check the Hardware page.

Do you have access to the console? Are a display and keyboard attached to the PC?

If so, enter “login” at the ha> prompt and at the Linux prompt, enter
dmesg --follow

wait until the messages have stopped and plug in the controller. Does anything appear on the screen?

Thanks for the suggestions. The power down and plug in did nothing for the hardware section, but this (pic below) is what happened when I did the “dmesg --follow” and plugged in. This is with the HomeSeer (I don’t have ZOOZ stick anymore). I’ll try to reach out to HomeSeer support tomorrow for some insight. Anything else worth trying in light of what we see on the screen here? Also how do I exit this screen? Lol

Control-C will get you out of dmesg now. You’ll need to work with HomeSeer on the errors you are getting. HAOS is not recognizing the Controller. If it isn’t added at the operating system level, you can’t proceed any further.

I just wanted to follow up here. I emailed HomeSeer support and the rep who responded said he has no experience with Home Assistant and I should ask the HA community for help on this. Honestly this makes sense because neither the ZST39 nor the G8 work on my HA instance. It’s not a G8 issue, it’s a HA issue (or maybe even a hardware issue).

At this point I just decided to return the dongle because everywhere I turn is a dead end. I’m mainly posting this so people will know that HomeSeer does not offer an ounce of support when it comes to Home Assistant.

Thanks for the help thus far, but I’m going to focus my efforts on something else for now and revisit this issue later.
