for the sake of knowledge, my issue was resolved by replacing my HUSBZB-1 with another one.
All the issues have disappeared since then.

I wanted to add this in case someone else had a similar issue.
I have the GoControl HUSBZB-1 stick and am running 0.81.2. I configured z-wave successfully via the gui, not the yaml file. I had trouble with zigbee/zha though. I was getting an error that there was an issue with zha when I signed in. I had this is configuration.yaml to begin with…

usb_path: /dev/ttyUSB1
database_path: /home/homeassistant/.homeassistant/zigbee.db
I changed the database_path to the following and it loaded. I have “zha.permit” and “zha.remove” in the Services menu now.
zha:
usb_path: /dev/ttyUSB1
database_path: /config/zigbee.db
I am not home right now, but will try and add a Osram light strip this evening!
