Cannot Update Home Assistant Operating System

I’m unable to update my Home Assistant OS, and I’m not sure why. The system notifies me of the need for an update, I run the process, the system reboots, but nothing changes. I’ve been having this issue for a year or so now. I’ve wiped my SD card and reflashed it to the newest version, starting totally from scratch, and it’s still not resolved. My system health output is below; I’m wondering if it’s a hardware age issue? I’m running a Raspberry Pi 3.

Precisely the same issue - same hardware too. But not sure the hardware is the issue. Been baffled for months.

I have the same issue on a Pi 4
