Cant adopt any Zwave devices with new ZWA-2

Background:
New house using existing HA setup, although I upgraded the ZWave controller to Connect ZWA-2. All the old ZWave devices and entities are no longer listed, but I cant get any of them to pair. even having a Monoprice plug 2 feet from the antenna won’t successfully adopt. I have tried the Exclusion process as well, but that fails.
I double checked the ZWave-JS and it is attached to the ZWA-2 which is plugged into a powered USB HUB.
Any thoughts on next steps?

OK, this was operator stupidity. I should have realized that as inclusion and exclusion wasn’t working a factory reset was needed.
