It seems that ZHA/zigpy is probably having problems talking to your Zigbee Coordination adapter, but not enough with that log, you are (always) also going to have to provide a description of your setup and what you are doing. E.g. what would someone else need to have and do to replicate what you see? For example, you did not write any details about your Zigbee Coordination adapter or how ZHA is connecting to it.
