GE/Jasco 14287 / ZW4002 Fan Speed Control Failures

For about 3 years I’ve had 4 of these operating flawlessly.

In the past two weeks, two of them report “Dead”.

How common is this? Is there an alternate preferred 3-speed fan control, or even better, is there a fan control that has a 0 - 100 % “dimmer” type control, like a rheostat that works with Zwave+ ?

I have a couple of those I’ve been testing with zwave_js, I had some issues with Home Assistant not tracking the correct state when I turn that fan off. E.g. HA shows on still, even though the fan is off. Have you seen this?
