Google Assistant | Unreliable (works 30%)

I am really struggling with reliability on google assistant integration.
I have started from scratch (thought I might have made a mistake somewhere).
It works about 30% of the time.

Duckdns, letsencrypt, docker 2024.1.3, ngnix proxy manager (internally address is DNS redirected to the proxy server)

Currently I am getting this error: ERROR (MainThread) [homeassistant.components.google_assistant.http] Request for https://homegraph.googleapis.com/v1/devices:requestSync failed: 500
but I have seen 404 also

I have set up the ENABLE LOCAL FULFILLMENT and it seems to work reasonably reliably on nest mini speakers atm on local network, altough it doesnt reliably update device state in the app.
