How do automations work, when do they fire?

I have a “smart thermostat” type of automation. Basically, a temperature sensor and a smart socket that controls the furnace.

My WHEN condition is, when temperature changes below 18 degrees

but it doesn’t always fire. sometimes it changes from 17.5 to 17.4 and it should fire, but it doesn’t.

but when i try to see traces, i run it manually and then it fires.

last run says it ran hours and hours ago but i can see the temp change in real time … so why doesn’t it fire sometimes?

how can i debug this further?

No i have not, much appreicated.

the ones i saw, they all needed a thermostat device which i don’t have but this one needs a sensor and a heater entities…

Yes go with the generic thermostat.
