It has pretty regular syncs (even while your phone is locked), shortcuts to support the syncs and an Apple Watch app to keep updating the data when the iPhone won’t provide it.

Your app looks very promising, but requiring a subscription just to sync my health data with Home Assistant? That’s a dealbreaker for me.
I’d be happy to make a one-time purchase, but I’m not willing to pay a monthly subscription solely to integrate my health data into Home Assistant.
It would be nice to create my own health dashboard, but honestly, it’s not important enough (I think for most other users as well) to justify a subscription.
If I understand the technology correctly, the app on an iPhone is needed to transmit the health out of Apple’s walled garden and into HA. I also understand that Apple is squeezing small and open-source developers with fees even giving away stuff through their App Store. It’s not unreasonable to charge a one-time fee to cover the download charges from Apple, but I agree on the subscription charge being a deal-breaker.
With that said, I’m not a potential customer of this app because I don’t use HA’s dashboards for anything…
