Is there a way to pop up a dialog with a form?

I’ve been using HA for quite some time now, but I’m still learning the concepts.

I was wondering whether it’s possible to pop up a dialog to the user with a form (preferably with multiple fields), have the user fill and submit it, and use the values later (basically, like a HTML form)?

I’m thinking specifically about a pop-up dialog with a submit button - not a card permanently displayed on the dashboard.
