Is there a straightforward way to migrate my Zwave and Zigbee devices from a SmartThings Hub to my HA server?

I don’t think there is. I think you need to delete the devices in SmartThings and then pair then in HA.
Having recently done this, be careful, remove the device from the Smartthings hub and then turn the hub off, I found it sometimes the ST hub will grab the device before HA can.
I think you mean to say to disassociate all devices individually. If you just remove them from the controller then you will have orphan devices which cause all sorts of problems with the network later on. With Aeotec they have a management utility which allows you to see the objects associated with the zwave controller at the hardware level and this helps to clean up any residual associations for devices which aren’t still available/dead, but you still have to factory-reset these devices if you ever recover/re-power them in the future. Maybe other manufacturer’s also have these tools.

Can I disassociate the devices and associate them to the HA dongles remotely? That is, does it require any physical acts at the devices?
Also, does this apply to zwave and zigbee devices the same way?
For ZWave, it’s a manual process; you need to put the controller into disassociate mode and then press the button on a device to disassociate it. Then repeat that for each remaining device.

Yes, exactly swapping over can be an issue, it took me a weekend to do all mine.
Stick with it, it’s worth is in the long run.
