Network UPS Tools - Forza UPS

I recently installed a Forza UPS to the NUT add-on. Had a little trouble setting up the configuration, but it ended up working. I am posting this in case it may help somebody.

My specific UPS was a Forza SL-801UL

And the configuration of the devices section of NUT:

the blazer_ups driver did not work for me.
