notify.mobile_app_xxs has stopped working on my (and my wife’s) iPhone 11Pro, iOS 26.1.
It’s working fine om my iPad pro, with iPadOS 26.1.
I have two installations - HAOS 2025.11.4. It fails on both. It also failed in 2025.10.4.
It’s hard to pinpoint exactly when it stopped working, but it’s not long ago (weeks?)
No settings on the iPhones have changed. HomeAssistant has “everything” enabled.
I installed the latest companion today. No change.
Nothing in the logs. When I run the action from dev.tools, it seems to be working well, but isn’t.
What could be wrong? Have I missed some breaking changes? Are our iPhones too old?

can’t help you to pin point the issue, but is working on iPhone OS 16 and HA v 2025.11.1. Probably your phones are not too old
