AFAIK the syntax is correct (2-space indent) and that is definitely the right path to the device (from the home directory).

does the user running HA have R/W access to the zwave stick?
Yes. The stick’s info:

Heh well I see one problem, your config says /dev/cu.usb.modem1411
but your ls
command says /dev/cu.usbmodem1411
