Siri always answers “done” or “ok” even if nothing happends.
I use
“He Siri Assist” then it asks “how can I assist”.
and I ask to turn on or off some lights,
this sometimes works, but if it doesn’t Siri still tells me “Ok” also if I just use some random words ik tells me “Ok”
Is is possible to get another notification if nothing happens?
Probably not. You need to look at errors logs and traces in HA to see if there’s a reason why the lights only turn on sometimes.
With Alexa, if you tell it to turn on the flapdoodle, it will reply “Sorry, I didn’t find a device called flapdoodle”. If it can’t access a known device, it will reply “XXX is not responding”. Otherwise it has no knowledge of failure within HA itself - which could be an error in HA preventing the turn on or an error in the device, which HA thinks is “on” - and will simply reply “OK”.

but I actually know why it only works sometimes,
either I use the wrong name and need to add some aliases, or I do not pronounce well enough what I want.
the issue is that there is no error response. for lights maybe not that much of an issue as you usually immediately notice that it didn’t work.
but if I want to use this for Central Heating or something a correct response is needed, because if I have to validate I can just grep my phone and enable it manually.
