I used to have an automation (I think but might have been a trigger) that would relock the front door after being unlocked for 5 minutes but quite some ago (months?) I removed that in favor of a door sensor that triggers an automation to lock the door 1 second after the door closes, which works fine. I’ve rececently noticed though that if I leave the door open/unlocked then after 5 minutes there is a Z-Wave lock.front_door_lock with alarm type 24 (locked by Z-Wave) and the deadbolt extends/locks. I tested enabling the door’s automatic relock and set it to 10 seconds and it relocks after 10 seconds with alarm type 27 so it’s not faulty firmware. I searched my config folder via grep and only see my current automation trigger by door sensor which has NOT fired. I am stumped!

not sure which smart lock you are using, but a lot of smart locks come with their own app & automation capabilities (independent from home assistant).
It might be that in that app there is a configuration that tells your lock to lock after 5 minutes.
This would have nothing to do with HA and you wouldn’t see that rule/automation anywhere in Home Assistant.
You would need to go into the native app of your smart lock and look for that rule and disable it there.

The lock is a Yale YRD210 and nothing other than HA was ever configured for/with it. Also, shortly after after acquring it (like a week) I purchased another for the back patio door and it works fine…when I’m home and awake it’s always unlocked and only locks when my night script is run going to bed or when I leave the house. As I said, I used to have an automation (or trigger) to relock the front door after 5 minutes but I removed that when I added the Zigbee door sensors on front, garage and rear patio doors. It’s like a phantom leftover that I can’t find…
grep is not giving the result if in automation it’s device ID. It depends how you search.
check historic and logbook when you see this again.
This model, which I also have, defaults to the 5 minute re-lock on the lock settings itself. You need to go into the settings using the keypad to disable it.

The autolock and it’s timeout are configurable via Z-Wave JS. The max timeout is 180 seconds, not 5 minutes. I set it to enabled and it autolocks after the configured timeout which defaults to 30 seconds by default. Disable it and I still get relock after 5 minutes which the logs show as triggered by Supervisor.
I renamed the entity from lock.front_door_lock
to lock.front_door
, edited the entity in all automations (thank you Spook) and the auto relock after 5 minutes is now not happening. I don’t know where it was and HA has 0 complaints about a misconfigured entity.
Are you maybe using other addons that can cause this? Such as NodeRED?
Perhaps they changed it on subsequent releases but I drove me nuts when I got my lock. And just because it says factory doesn’t make it so. You are trying to track down a gremlin and don’t have any explanation - that bears a look at this setting to see if it is actually enabled, even if you think it’s not.
