Trying to add Custom Compact Header with the new version

I didn’t have it installed previously.
So for me it looked quite simple.
I added it via HACS.
I added the include lines in the lovelace.yaml.
I do see the CCH Menu item, when I click it, I just get a black bar:

What did I miss?
