Z-Wave inclusion: Node detected but not added

Wondering if anyone can help me or maybe point me in the right direction. I’m trying to add a node to an existing network. It is the same device type as one added previously and functioning OK.

I clicked the Z-Wave add node button, and put the node into inclusion mode. I see the following in the log:

I understand Node001 is the controller. It seems to be talking to the node I am trying to add, but I not see it in Home Assistant. Why is it telling me there is “nothing to do”? Is there something I am missing?

That log message just indicates the controller is in inclusion mode but nothing has been added. You need to hit some buttons on the device to get the inclusion started.

Yes, I just noticed that node001 is the controller. I’m pressing the button to include the node, but it is being ignored for some reason.

I’m pressing the inclusion button on the node. The instructions tell me I should look for a single orange flash on the node, which I am getting. But it just will not add.

What is “Node 2” at

Edit: Node 2 is the existing sensor

Why is it telling me the “Expected reply was received”?

I can see the LED on the new device flashing green, but its nowhere to be seen on Home Assistant.

As far as I can see the nodes are responding as expected. Can anyone suggest anything? Diagnostics? Z-Wave maps? mqtt to zwave? Remove Z-wave and start again? Delete config files?

I restarted Home assistant and rebooted the host. Nothing.

Hold the phone, I have a bunch of messages in the log about node 3!!!

For example:
