Zooz ZEN15 instantly factory resetting upon connection?

I’ve got a Zooz ZEN15 switch here along with a Zooz 800 stick.

I am connecting the switch via QR Code, but the instant it connects to HA it goes dead, and I get a notification that " Power Switch has been factory reset and removed from the Z-Wave network."

I’ve done nothing to manually remove this switch, everything here is brand-new out of the box.

Any thoughts on what I’m doing wrong here?

Can you try adding it “manually” when the hub is in inclusion mode, instead of with the QR code? I have one of these but do not believe I used the QR code to add it. Also, is there any chance the button is stuck in? Holding for 20 seconds appears to be how you can reset it.

Thanks for responding! by “inclusion mode” do you mean when HA is scanning for devices like so?

I wonder if I’m dealing with a bug in the zooz 800 stick, because it’s being really flaky. Right now the power switch shows up 9 times, all of them are dead.

It’s still trying, because it keeps adding more, and as soon as one shows up i can actually use it to power the switch on and off and view consumption, but 5 seconds later it’s dead again.

Well, I sorted the issue.

I dug through the debug logs and saw that it was adding the switch, gathering some info from it, then pinging it again and not getting a response back in time. The system would then declare the device dead. I guess sometimes it was still in provisioning mode, so it would repeated the process until the provisioning timeout ended.

I thought the issue might be latency or some weirdness with USB devices and the Docker container, so I spun up an HA rPi and moved the stick over to that, and everything has worked well since.
