It doesn’t appear to be possible to actually factory reset the device, at least from following their instructions. I’ve tried every permutation possible - cover on, 10 clicks. Cover off, 10 clicks. Cover off, cover on, 10 clicks. None of it works. The manual claims that the LED should flash multiple times after a successful reset. It does not do this.
I discovered this when I had to reset my zwave stick and attempted to re-add my devices. However, it IS possible to exclude it:

and then pair it. So it’s not completely broken. Just figured I’d share in case anyone else ran into the same problem.
I have two of them. Can’t figure out how to factory reset them. Tried the 10 clicks as well. Nothing. I don’t have my old z-wave controller so looks like these are garbage.
You don’t need the old controller to exclude them. The exclusion command just tells the device to forget any hub it was connected to previously, from what I understand.
Still, regardless - I couldn’t get them to actually send the “motion detected” signal at all, even though they were connected. Garbage. Swapped them with a cheap $20 sonoff motion sensor that worked perfectly right off the bat and responded faster, too.
I had the same issue. I found the solution. The tampered switch was pressed too far in, and it was not springing back. Basically, the switch always though the cover was on. And it seems the firmware needs to see the tamper switch OPEN for it to be able to to Factory Reset.
I gently pried back the tamper switch - its a little metal flap on the inside of the sensor. After that all worked as per reset instructions.
