I had the same issue with the add-ons not starting after migrating from an SD card to an SSD drive and restoring from a backup. Rebooting the the machine fixed the issue. Thank you!
same here - upgrade HAS OS, then AddOns “not starting”. Actually - they do start! All their log files are fine, Zigbee2MQTT simply works. It is just their state that is wrongly reported (as “unknown”, checked in an SSH session), so the user i/f thinks you need to start them. Only a complete server shutdown solves the issue, not simply a restart, as reported above.
I have just changed the solution using the mod tools, hope that helps!

System SHUTDOWN (not RESTART) did the trick for me. RESTART does not fix the problem. Did NOT need to delete v2.d database

I had the same problem when I moved from an SD card to a USB SSD on the PI4 and restoring the backup. All addons were running but just not accessible from the Homeassistant UI. It would report that the addon is not running.
Just rebooting from the settings - system menu didnt work.
I had to perform a shutdown instead of reboot , disconnect the power to the PI4 and re connect it.
Everything works fine now.

So I had this same issue. Rebooting / Shutting down worked for the most part. But for some reason Node Red still has the same problem. I can press the start button, and it’s appearing in the sidebar, but it still says “Add-on is not running. Please start it first.” Even from its own Add-On page, if I press the “Go to Web UI” button it wll return “502: Bad Gateway”.
Google Backup and VSCode are opening though.

Hard reboot worked. Reboot from within the menu did not work. Had to remove power after shutdown to get things working.

Another happy customer here. I needed to restore last night’s backup and afterwards nothing would start. Shut the machine down and unplugged for a few seconds, and on return they were all working.
This worked for me for Home Assistant Yellow. Thank you!
Had same issue here after a restore from an RPI4 to an Proxmox installation.
Addons seemed to be started and in the logs you could see that they were running but the supervisor gave status “unknown”.
Reboot did not fix the issue.
Issue was resolved by shutting down the machine completely waiting a few seconds an powering it on again. After that the addons were reporting the correct status again.
