Alright, then I am out of ideas. I am assuming you tried power cycling the ATV.

Just unplug and plug. It showed standby for a second then turned to ‘idle’ when it’s ON. Afterwards I put it to sleep and the status stayed as ‘idle’.
I think I might figure out where the problem is. I have always set ATV audio output to Homepod. The integration cannot work with that. I did a test to change it back to TV speakers and the states of ATV responded better.
Great! Don’t have Homepods myself so haven’t had the issue.
I’ve been having the issue where my Apple TV 4k 2nd gen won’t report status after going turning it off and it goes into “standby” state. The work around I figured out, that has been working for me, is to reload the AppleTV integration whenever my TV goes from on to off. Reloading the integration causes the state to report correctly.
I’m also having this problem for some while now. I had the same idea as you but i can’t pin it down when it happens. My workaround was similar to yours. But I reload the integration 10 seconds after any event happens. The integration reloads super fast so it’s barely noticeable that there is downtime.
