Good morning, I have the same issue. I followed along with the steps above and added the device to Zwave JS UI and can see the device there. I have two door locks. One I added two years ago and now I am adding another one. The Main Door Lock is Node 2 and has been active and working for a while. Now, Node 7 and the Office Door lock I only see the same entities as the OP. I have some pictures and a log enty added.
Love to know if any one has any idea. I stop Zwave JS and start Zwave JS UI, added the door lock and compelted the interview. I stop Zwave JS UI and restart Zwave JS and the new door lock is there but I cannot control it like the other one.
