I have a zwave switch that seems to be turning itself on. This is the 2nd time it has happened. I can physically look at the switch and it is ON. Looking at home assistant and the history, home assistant indicates that the switch is OFF and has been off for 24 hours straight and was last used 2 days ago.

I searched the zwave log file and I don’t see any entries for node 69 which is the switch that is having the issue. I don’t see anything in the home assistant log file either. Any ideas for debugging? This is a huge issue and is a whole new level of unreliability. Dangerous would not be an overstatement.
I also found that the zwave switch was physically ON and home assistant showed it as OFF. I then walked over to the switch (that was physically ON) and pressed the “up” paddle to turn the switch ON even though it was already ON. When I did this, home assistant updated from OFF to ON for the switch.
